Fee Transparency
At Community Care Network (CCN), we believe everyone deserves access to high-quality care. To make this possible, we offer income-based adjustments on our services. This guide lists the services we provide, their costs, and how your payment amount is determined. If your household qualifies, your portion of the bill may be adjusted based on your income.
(Note: The estimated payment amounts in this guide apply to self-pay patients. If you have health insurance, your out-of-pocket costs may vary depending on your specific coverage, co-pays and deductibles. Please contact your insurance provider for more details.)
How Much Will I Pay?
We use your total annual household income and family size to determine what percentage of the full cost you’ll pay. You may be responsible for between 15% and 100% of the total cost. If your income is above the threshold, you’ll pay 100% of the charges.
Example:
If the standard charge for a service is $184 and you’re responsible for paying 30% of the cost:
- Multiply the full charge by the percentage you’re responsible for: $184 × 0.30 = $55.20
- Your cost would be $55.20 for that service.

- Determine Your Total Annual Household Income: Look at your total annual household.
- Find Your Family Size: Count how many people live in your household.
- Using the Calculator below: enter the information from steps 1 & 2, then select the service you need.
Understanding Service Levels
The level of service you need (e.g., for medication management or medical evaluations) is determined based on the complexity of your care needs. Here’s how it works:
- Initial Assessment: A healthcare provider will evaluate your situation during your first visit. This includes understanding your medical history, current medications, and care goals.
- Service Level Assignment: Based on your assessment, the provider will assign the appropriate service level:
- Level 1: Basic care or minor adjustments.
- Level 2-3: Moderate needs, such as new prescriptions or follow-ups for ongoing treatments.
- Level 4-5: Complex needs, including managing multiple medications or significant health concerns.
- Discuss with Your Provider: During your visit, the provider will explain why a specific service level is recommended and answer any questions about costs.
