Helping Families Access Essential Services

Navigating Health Care with Support

For children with developmental disabilities and their families, navigating health care can feel overwhelming. As an Early Periodic Screening, Diagnosis, and Treatment (EPSDT) service, The Bridge Program offers care coordination to help families access and coordinate essential:

  • Medical services
  • Educational resources
  • Social services
  • Community support programs

Who Can Participate?

  • Children and young adults under age 22
  • Individuals who meet Vermont’s criteria for developmental disabilities
  • Vermont Medicaid recipients, as determined by the Department of Vermont Health Access (DVHA)
  • Children not receiving care coordination, case management or service coordination from other AHS-funded sources listed in the Bridge Program Guidelines

Note: Children currently receiving care coordination through other state-funded programs may have different options available.
