Supporting Independence in Residential Settings

Home Supports are services, supports and supervision provided for individuals in and around their residences up to 24 hours a day, seven days a week (24/7). Home supports help individuals acquire and retain life skills, to maintain health and safety, and develop as much independence as possible in their home.

Home modifications for disability-related accessibility needs may be provided, along with cost-effective technology that promotes safety and independence in lieu of paid direct support.

Supervised Living Support

Regularly scheduled or intermittent hourly supports are provided to individuals living in their own home or with family members. This personalized support model offers:

  • Flexible scheduling based on needs
  • Support from trained professionals
  • Skills development
  • Independence building
  • Health and safety monitoring

Support may be provided by Community Care Network Developmental Services staff or by workers hired through a shared living provider or family.

Staffed Living

24-hour support in a home setting for one or two people.

Group Living

24-hour staffed support at Community Care Network's licensed, Level 3 group home in Rutland City. This home environment supports 2-4 individuals with individualized and comprehensive plans to meet:

  • Personal care needs
  • Health requirements
  • Social engagement
  • Daily living needs
  • Includes nursing oversight

Shared Living

A support model where individuals with developmental disabilities share a home with dedicated caregivers who offer:

Options range from basic "roommate" supports to intensive supports for complex needs, which may include:

  • 2:1 supports when needed
